    Possible Scenario:

    After the BHA has been pumped down, connection to the tool string can be lost due to various issues. After multiple attempts to reconnect without success, the BHA should be pulled out of hole for inspection. 

     

    Analysis:

    Once the BHA has been pulled out of hole and rendered safe, inspection should occur. In this case, the issue is occurring at the cable head, so the entire tool string should check good after testing with the verifier box. When a new rehead  is being installed, cable head issues such as a loose tear drop can cause intermittent communication issues. 

     

    Resolution:

    After identifying the problem, the issue on the cable head should be addressed and emphasized to the entire crew so future mistakes like this can be avoided. This highlights the importance of checking all equipment after maintenance, repairs, or replacing parts.
